Research, Mentors, and Academic Currency

Part of Medical School. Research became more valuable to residency programs at exactly the moment Step 1 stopped producing a number, and access to it runs almost entirely through people. This is how projects and mentors actually get handed out, how authorship is decided, and what to do when you do not already know anyone.


The short version

Research functions as a currency, and the exchange rate went up. When Step 1 went pass/fail, programs lost a sorting number and leaned harder on everything else they could count. The Step Exams covers that shift.

What gets counted is items, not discoveries. Abstracts, posters, and presentations are listed alongside publications, and a single project can produce several.

Access is a relationship problem before it is an academic one. Who gets into a lab, who gets a mentor, and who ends up on the paper are decided in conversations, not applications.

Authorship is negotiable, and the time to negotiate is before the work. The criteria are published.

A dedicated research year is a separate and much bigger decision, and it is a money decision as much as an academic one. The Year That Costs an Attending's Year has that arithmetic.

What counts as research here

A chart review, a quality-improvement project, a case report, a systematic review, survey work, education research, and bench science all appear in the same section of a residency application. Programs in different specialties weight them differently, and the ones that most want research generally want it in their own field.

The application counts discrete items: peer-reviewed publications, articles submitted or in press, abstracts, posters, and oral presentations, each listed separately. That structure matters more than it should, because one competent project presented at a regional meeting and later written up can appear as several countable lines, while a year of careful bench work that has not yet produced anything appears as one.

The national picture is published. The NRMP reports, by specialty and applicant group, the counts of research, work, and volunteer experiences that matched and unmatched applicants actually had.1 Read the table for the specialties you are considering before you conclude you are behind, and read it again after, because the ranges are wider than the reputation.

The counts have inflated over several cycles, and the inflation is partly an artifact of how items are entered rather than of students doing more science. And a long list of thin items is legible as a long list of thin items to anyone who reads it closely, which is often the person interviewing you.

The FLI version: access runs through people you were not born knowing

This is where first-generation and low-income (FLI) students start furthest behind.

Getting into a project in medical school almost never begins with an advertised position. It begins with somebody who already knows you suggesting your name, or a faculty member with a project remembering you from a small group. Students who arrive with a physician parent, a family friend in academic medicine, or an older sibling two years ahead start with a warm introduction and a working model of how these conversations go. Students who do not, wait for a posting that is never coming.

The gap shows up again inside the project. Knowing that you can ask about authorship, that you are allowed to decline unpaid work you do not have time for, and that a mentor who never answers email can be replaced are all things people learn by watching someone do it.

None of this is a reason to be discouraged, because the whole system is made of learnable moves.

How a project actually gets started

Start from a person, not a topic. In medical school the constraint is rarely which subject interests you; it is who has an active project with room on it and a habit of finishing things. Pick the person first and the subject second, at least for your first project.

Ask the class above you who is good to work with. They know who publishes, who answers email, who puts students on papers, and who has three abandoned chart reviews with a student's name attached. This single conversation is worth more than any faculty directory.

Then write the email, and make it short. Two or three sentences on who you are and what you have read of theirs, one clear ask, and your availability in hours per week. Attach nothing. Send it to one person at a time, and follow up once after ten days. The cold-email anatomy in Research still applies here; the difference is that you now have a school email address and a shared institution, and both raise the reply rate substantially.

Ask for a defined piece of work. "Is there a piece of an existing project I could take on this block?" gets answered far more often than "can I do research with you." A defined ask is easy to say yes to.

Use the machinery your school already has. Most schools run a summer research program between first and second year, and many pay a stipend. That stipend is often the only way a student without family support can afford to spend a summer on research rather than on paid work, and applications usually close in the winter of first year, months before anyone thinks to look.

Two different people, doing two different jobs

Students hear "mentor" as one role. In practice you need two, and they are rarely the same person.

Someone who teaches you. Answers questions, reads your drafts, tells you when the analysis is wrong. Often a resident or a junior faculty member, often the person you actually work with day to day. This relationship is the one that makes you better.

Someone who advocates for you. Says your name in a room you are not in, calls a program on your behalf, writes the letter that carries weight because of who wrote it. This is usually a more senior person in the field you are applying to, and the relationship is built through sustained visible work rather than through meetings.

They are sometimes the same person and usually not. If every relationship you have is the first kind, you will be well taught and under-advocated for, and that shows up in the fourth year when letters are due. Keep the relationship alive with short, specific updates: what you finished, what you need, what is next. Three sentences every few weeks. Mentorship decays from silence more often than from conflict.

Authorship, and the conversation to have before you do the work

The International Committee of Medical Journal Editors publishes the criteria most medical journals use. Authorship requires all four of: a substantial contribution to the conception, design, data acquisition, analysis, or interpretation; drafting or critically revising the work; final approval of the version to be published; and agreement to be accountable for the work. Contributors who do not meet all four should be acknowledged rather than listed as authors. Order is decided collectively by the author group, not by the journal.2

If you only collect data, you have not yet met the criteria, and the standard fix is to be part of the writing. Ask for that explicitly, at the start: if this becomes a paper, what would I need to do to be an author on it? Asked at the beginning, that question is normal and most mentors answer it straightforwardly. Asked at the end, it is a complaint.

Write down what you agreed. A short email after the conversation summarizing the plan protects a student far more than it protects a professor, and it costs nothing to send.

If you are left off a paper you believe you earned, ask the corresponding author directly and in writing first. If that fails, most schools have a research integrity office or an ombudsperson, and this is what they are for. Students in the weakest position socially are the ones most likely to absorb it quietly, which is precisely why it keeps happening.

When a project goes nowhere

Most do not end in a paper, and that is normal rather than a verdict on you.

A project that stalls still leaves you with things worth having: a faculty member who can describe your work, a method you now understand, sometimes an abstract you can submit to a regional meeting on your own. Submit the abstract. Regional and specialty-society meetings accept far more than national ones, student registration is discounted, and travel awards exist and go unclaimed.

Know when to leave. If a mentor has not answered two emails and a project has not moved in three months, you are allowed to start something else. Do it without burning the relationship, because the field is small, but do it. A dead project you stay loyal to costs a year; a project that produces a negative result costs nothing but the write-up.

Two moves make most of the difference, and both are available in the first year. Ask the class above you who is good to work with, and ask the authorship question at the start rather than the end. The system rewards students who already knew the moves, and the only reason it looks like talent is that the moves were taught at somebody's kitchen table instead of in a classroom.
