The catch that dominates every honest conversation about PHM: as of 2019 it requires a 2-year fellowship to do a job thousands of pediatricians used to walk into straight out of residency.

What they actually do

Pediatric hospitalists are the generalists of the children's hospital. They provide comprehensive inpatient care for hospitalized children, from the bread-and-butter acute admissions (bronchiolitis, asthma exacerbations, pneumonia and other infections, dehydration and gastroenteritis, the febrile infant) to medically complex children with multiple chronic, often technology-dependent conditions.1 A large share of the work is co-management: providing the general-pediatric medicine for a child admitted primarily under orthopedics, neurosurgery, or another subspecialty, so someone is always minding the whole child rather than one organ. Many hospitalists also staff the newborn nursery and, depending on the site, attend deliveries or cover the NICU. And a defining, unusually large piece of the role is non-clinical systems work: quality improvement, patient safety, throughput and operations, clinical-pathway development, plus teaching residents and medical students on the wards.1

The scope varies enormously by setting, and this is central to understanding the field. At freestanding children's hospitals, hospitalists spend the majority of their time on inpatient pediatrics (a survey found a median ~66.5% of time); at community hospitals, inpatient peds may be a much smaller slice (median ~10%), blended with nursery, ED, or outpatient duties.1 Roughly 30% of self-identified hospitalists practice hospital medicine exclusively, ~31% combine it with general pediatrics, and ~39% combine it with another subspecialty, a reminder that "pediatric hospitalist" describes a spectrum of jobs rather than one.1

Representative procedures: PHM is a moderate-procedure generalist field, not a procedure-heavy one. Commonly: lumbar puncture · procedural/moderate sedation (where hospitalists run sedation services). Setting-dependent and less universal: central line placement · intubation/airway management · chest tubes · incision & drainage · bladder catheterization/suprapubic aspiration. The advanced airway/access procedures are more typical of community solo-coverage roles; at large academic centers, PICU and subspecialty teams usually do them.1

A block in the life: PHM runs on scheduled clinical blocks rather than an open-ended clinic day, commonly a 7-on/7-off–style rhythm (a week of day service, then a week off) or rotating day- and night-service blocks; night coverage (dedicated "nocturnist" shifts at some sites, everyone rotating through nights at others) is a defining feature.2 A service day is morning rounds on the inpatient team (often family-centered bedside rounds with residents and students), new admissions from the ED or clinic throughout the shift, discharge planning, ongoing family communication, cross-cover of the service, and responding to rapid-response calls and deterioration on the floor. In many jobs, daily newborn-nursery rounds are part of the block. On non-service (or "off-service") weeks in academic jobs, the time goes to QI projects, committee and operations work, teaching, and scholarship. The core contrast with clinic-based pediatrics: you trade longitudinal outpatient continuity for episodic, high-throughput, team-based inpatient care with a heavier night/weekend load, but real, protected "off" blocks.12


The training path & time to completion

Medical school (4 yrs) → general Pediatrics residency (3 yrs) → PHM fellowship (2 yrs) → board-eligible with the American Board of Pediatrics (ABP) in Pediatric Hospital Medicine.3 This is a two-step entry: you match into pediatrics residency straight from medical school, then apply again during residency (in the PL-3 year) to fellowship through the NRMP Medicine and Pediatric Specialties Match, the "fall match," with results released in December.3

  • Fellowship (2 yrs), and note it's short. The standard PHM pathway is two years, one of the shortest fellowships in pediatrics (most peds subspecialties are three). A 3-year research-heavy pathway also exists. Training must be in a program accredited by the ACGME (US) or the Royal College of Physicians and Surgeons of Canada; part-time training is permitted (up to 4 years for the 2-year pathway).3 The fellowship blends inpatient PHM service, community-hospital and newborn/nursery rotations, sedation, complex-care, electives, and a substantial scholarship/QI project requirement.3
  • Board: the American Board of Pediatrics (ABP), through a sub-board in Pediatric Hospital Medicine, which requires prior ABP general-pediatrics certification, completion of the accredited fellowship, and passing the subspecialty exam.3
  • Total from the start of college: ~13 years (4 undergrad + 4 med school + 3 residency + 2 fellowship). That's actually shorter than most peds subspecialties (which run ~14 years on a 3-year fellowship), and the 2-year fellowship is one of PHM's few timing advantages.3

The newest subspecialty in pediatrics. PHM is worth situating in time: the American Board of Medical Specialties approved it in 2016, and the ABP administered the first certifying exam in 2019, decades after older peds subspecialties (pediatric critical care's first exam was 1987).4 That recency is the root of the ROI and pipeline debate below.


How competitive is it?

Because PHM is a subspecialty, "competitiveness" works differently than for a base residency. The accessible front door is general pediatrics residency, one of the more open routes in medicine, and an unusually IMG-friendly one: 30.4% of its filled positions went to international graduates in 2026 against 25.2% across all PGY-1 positions, while its 21.1% DO share sits at the 21.5% all-specialty average rather than above it.5 The selectivity, such as it is, shows up at the fellowship step, and here PHM is best described as attainable but genuinely subscribed: a well-filled fellowship rather than an under-filled one.

Most recent cycle (2025 Medicine and Pediatric Specialties Match; appointment year 2026):6

  • PHM filled 86.7% of its positions. That was 124 of 143 offered, across 83 programs (69 of which filled every spot, 83.1%). That's above the ~78.3% average fill rate across all pediatric subspecialties that year, which makes PHM one of the higher-filling, more competitive peds fellowships rather than an under-subscribed one. (For context, in the appointment-year-2022 match PHM filled 100% of positions.)67
  • It's still attainable if you want it. Roughly 137 applicants preferred PHM for those 143 positions, so the pool and the seats are closely matched and the large majority of applicants who rank it match. The hard part of the path is building a strong pediatrics-residency application rather than squeaking past an ultra-selective fellowship.6
  • PHM runs 83 programs, third among pediatric subspecialties, behind neonatal-perinatal medicine's 104 and pediatric emergency medicine's 99 in the same match. For a subspecialty that had essentially no accredited programs before 2019, that is a build-out from zero to the top of the field in six years: ACGME's program series for PHM runs 54 · 65 · 68 · 73 · 80 across AY2020-21 to AY2024-25, up 48.1%, and on that count PHM is second among pediatrics-sponsored subspecialties, behind neonatal-perinatal medicine's 110. It's one of the fastest fellowship expansions in pediatrics history, fueled by demand for the new required credential.8
  • Composition of matched fellows (2025): predominantly US MD (~80.6%, 100 of 124), with ~14.5% US DO (18) and only ~4.8% IMG combined (6: US-citizen IMG 3.2%, non-US-citizen IMG 1.6%). So the fellowship cohort is meaningfully DO-inclusive but only modestly IMG-represented. The real accessibility of the PHM path is the open pediatrics-residency front door rather than an IMG-heavy fellowship.9

The honest read: PHM is attainable rather than a gauntlet, and well-subscribed rather than wide open. Most who rank it match, yet it fills above the peds-subspecialty average, and the matched cohort is mostly US MD/DO. The abundance of programs reflects explosive build-out to meet the new credential; it has not translated into lots of empty seats.


Compensation — the robust version

PHM pay is genuinely under-documented, and it's important to say so plainly rather than pretend to a precision that doesn't exist. There is no authoritative, widely-cited single "pediatric hospitalist national average" the way there is for adult hospitalists or peds cardiology. The largest comp report (Doximity) doesn't break out pediatric hospitalists as a line; MGMA, the benchmark PHM job offers actually reference, is paywalled. So the number has to be bracketed from aggregators of varying quality, and every point estimate should be held loosely.10

The honest national number. The public aggregators disagree by structure, not just noise:10

Reading across them, a defensible "typical full-time" figure is ~$230,000–$300,000 total compensation, meaning hospitalist-level pay clustering near or modestly above the ~$265k general-pediatrics benchmark rather than the six-figure premium of the acuity-driven peds fields. The numbers real offers are built from live in MGMA and the AAP Section on Hospital Medicine salary survey, and neither is public.10

Where PHM sits among peds subspecialties. Using Doximity's 2025 peds lines as the map: neonatology ($355k) and peds cardiology ($352k) sit at the top, alongside peds critical care at $355k, which is not a Doximity line and comes from Marit Health's self-reported panel; peds GI ($298k) and pulmonology (~$282k) in the middle; the cognitive subspecialties (nephrology ~$263k, heme/onc ~$256k, ID ~$248k, rheum ~$232k, endo ~$230k) at the bottom, with general pediatrics at ~$265k.11 PHM lands in the lower-middle, effectively at the general-peds line. The telling contrast: unlike PCCM and neonatology, which out-earn general peds because of in-house 24/7 acuity, PHM does broadly similar shift and coverage work but hasn't historically translated it into a large pay premium.11

Starting vs. experienced: a compressed curve. Tenure moves PHM pay less than in RVU-heavy fields; the biggest swing factor is setting and call/night/shift burden, not seniority. Shift, night and weekend differentials plus moonlighting are a meaningful share of total pay, and they move it more than tenure does. This page used to size that gap with a Glassdoor "additional pay" band; Glassdoor is employee self-report with no disclosed panel, and no survey prices PHM differentials separately.1012

Academic vs. community. PHM is overwhelmingly employed, at academic children's hospitals plus hospital-employed community groups; true private practice barely exists (you can't run an inpatient census solo). The general pattern: community/hospital-employed roles pay more cash; academic roles trade cash for teaching/research protected time (and often loan-forgiveness eligibility).10

Geography. Consistent with every peds subspecialty: higher pay in lower-cost, high-demand, and rural-adjacent markets; compression in academic-dense, high-cost coastal metros where children's-hospital salaries dominate. ZipRecruiter's top-paying metros cluster in high-demand/lower-supply areas rather than the biggest cities. A clean PHM-specific urban-vs-rural dollar delta isn't published (limited data).10

Moonlighting is the real income lever. Given a modest base, extra shifts, cross-coverage, and locums are how many hospitalists raise total pay, a genuinely accessible lever because the work is shift-shaped. The hourly rates quoted for locum pediatrics come from staffing agencies quoting their own placements, which this site excludes as a seller publishing its own price, so no rate is given here.12

The trend that colors all of it. PHM tracks general peds, with slow, inflation-adjacent growth and no procedural upside, while a structural headwind looms: closures of pediatric inpatient units in community hospitals are consolidating peds inpatient care into large academic centers, which erodes the higher-paying community roles and concentrates jobs where pay is more compressed.13 Net: flat-to-modest real growth, and the bigger economic story is the fellowship requirement below rather than year-to-year salary drift.1013


The ROI & pipeline controversy — the defining story

This is the single most important thing a premed should understand about PHM, and it deserves its own section because it shapes everything from who enters the field to how fairly the door is held open.

What happened. For roughly two decades, pediatric hospitalists practiced with no fellowship, since a general pediatrics residency was enough. The 2016 ABMS approval and 2019 first exam created a new 2-year fellowship credential that is now the standard route to board certification.14 In effect, PHM turned a job people did straight out of residency into one that now (for board certification and most academic jobs) requires two additional years of training.14

The ROI argument against. The fellowship adds two years of low fellow-level pay, roughly the general peds-subspecialty stipend, in exchange for attending pay that is only near or modestly above general pediatrics. The opportunity cost is real: two years of forgone attending salary (on the order of several hundred thousand dollars in lost earnings and deferred savings) for a pay bump over the no-fellowship baseline that is small, a genuinely marginal and for some negative financial return. No dollar-figure ROI analysis has been published for PHM, so the size of that gap is a judgment call rather than a number you can quote.14

The pipeline-equity concern, and why it matters for FLI/IMG trainees. Critics argue that a mandatory low-paid fellowship narrows the pipeline and disproportionately disadvantages trainees with debt, family financial obligations, or caregiving duties who can't afford two more low-income years or can't relocate for fellowship, a group that includes many first-gen, low-income, immigrant, and IMG physicians, with unclear evidence that patients are meaningfully better off. It's the classic "credential creep vs. professionalization" debate, and PHM is its most current example.14

The argument for. Supporters counter that the fellowship standardizes training in quality improvement, medical education, complex/medically-complex care, and scholarship, and gives the field a clearer professional identity and academic footing rather than leaving hospitalist competency to on-the-job accretion.14

The crucial nuance: community jobs still exist without it. The labor market still has many community hospitalist roles open to general pediatricians without a PHM fellowship. The fellowship is required for board certification and academic positions, but a pediatrician who wants community inpatient work, often near family, may not strictly need it. There was also a temporary "practice pathway" (the 2019, 2021, and 2023 exam cycles) that let established hospitalists certify on practice experience without fellowship, and that window has now closed, so fellowship is the primary route for new entrants going forward. That is exactly why the ROI question matters more now than it did five years ago.14

The honest bottom line, laid out without steering: whether the fellowship is "worth it" depends almost entirely on the job you want. For an academic, credentialed, QI/education-leadership career, it's the standard price of entry and buys real training. For community inpatient work near home, it may be two years you don't strictly need. That's a genuine fork, and it lands hardest on exactly the students this site is written for.


Lifestyle & the shift-work bargain

The most-cited pro of PHM: when your block ends, you hand off the entire service and you are genuinely off: no continuity panel, no inbox chasing you home, no personal pager between blocks.2 That clean, finite off-time is the single biggest lifestyle contrast with clinic-continuity pediatrics, and it's the reason many generalists choose the hospital over the clinic. Blocks are scheduled far ahead (7-on/7-off–style rhythms are common), so you often know months out when you're off, and off is really off.2

The most-cited con, and it's the same coin: the hospital never closes. Nights, weekends, and holidays are structural and permanent for your whole career rather than just training, whether through dedicated nocturnist shifts or everyone rotating through nights.2 What PHM generally avoids, relative to the ICU peds fields, is continuous 24-hour in-house call; much of the work is built around defined ~10–13-hour shifts. But circadian disruption from night blocks is a real, not cosmetic, cost.

Workload is real and openly debated. A national survey of community PHM programs found a median of ~1,882 clinical hours/year for a full-time hospitalist, with community-employed physicians working more hours and carrying a higher census than university-employed peers. Tellingly, only ~63% of program leaders rated their own program's workload as sustainable. That's an unusually candid self-assessment from a young field still writing its staffing norms.7

"If you've seen one PHM program, you've seen one PHM program." Schedule, census, night model, and independence vary dramatically hospital to hospital, which is genuinely a feature: you can often find a schedule shape that fits your life if you're willing to be geographically flexible.2

Lifestyle rating: 4/5. Among the more livable acute-pediatric paths, on the strength of clean, finite off-time and no clinic panel or between-block pager. Held back from 5 by permanent nights/weekends/holidays and the circadian cost of night blocks. (For contrast: outpatient peds trades nights away for a never-ending inbox; PCCM/neonatology trade even harder nights and 24-hour in-house call for higher acuity. PHM sits in between, at bounded shift work and generalist acuity.)


Wellbeing — the part to take seriously

Burnout: no PHM-specific number exists, and this page does not invent one. PHM is not broken out as its own line in the major national burnout or compensation reports, so its wellbeing picture has to be read through a parent, and there are two of them. Medscape's 2024 report puts general pediatrics at 51%, just above that survey's 49% all-physician average and inside its ten highest-burnout specialties, though well short of emergency medicine's 63% and OB/GYN's 53%. The other candidate is adult hospital medicine, and no freely readable table publishes a row for it at all: the relays that carry the Medscape report print only its ten highest and ten lowest specialties, and hospital medicine appears in neither list. So this page reads general pediatrics, which the work supports as well as the data does: the patients, the wards, the children's-hospital staffing model and the payer mix are pediatric, and what PHM borrows from adult hospital medicine is the shift structure rather than the population. The free primary instrument does not reach this field either: AMA's 2025 Organizational Biopsy names fifteen specialties and neither pediatrics nor hospital medicine is among them, which is why everything here is Medscape. Read PHM as around or slightly above the physician average, with real uncertainty, and treat it as a genuine data gap rather than a measured figure.15

What the PHM-specific evidence there is says. A four-hospital study of moral distress found pediatric hospitalists reported significantly lower moral distress than residents (mean composite 83.0 vs. 97.6). The most common driver was excessive documentation requirements; the most intense was working with abusive patients/families; and weekly burnout tracked strongly with higher moral distress.16 The usable takeaway: attending hospitalists carry meaningful but not extreme distress, and the biggest fixable driver named is administrative and documentation load rather than the clinical work itself.16

Bounded hours help; shift work and rising acuity hurt. The wellbeing case for PHM is that finite blocks and a clean handoff protect against the open-ended inbox/"pajama-time" load that grinds down clinic pediatricians. The case against is that night/weekend coverage, circadian disruption, and rising inpatient census and acuity are real, documented stressors. Both are true at once.

Satisfaction & would-choose-again. No clean PHM-specific figure exists, another gap. Pediatrics broadly reports high meaning even where pay lags, and hospitalists frequently cite variety, teamwork, teaching, and systems-improvement work as durable sources of fulfillment.115

Career longevity: the night-shift question. The openly discussed concern is whether rotating night blocks are workable into one's 50s and 60s; circadian toll compounds with age. The upside is that because PHM roles are so varied and abundant, real off-ramps exist without leaving the field. Hospitalists shift over time toward day-weighted schedules, nocturnist-free roles, reduced clinical FTE, or non-clinical work (QI, education, administration, leadership). You can evolve the job rather than abandon it.


Who's in the field (demographics)

  • Women: 82.6% of pediatric hospital medicine fellows are women (195 of 236, AY2024-25), among the highest shares of any GME field. That runs ahead of both the parent field's residents (74.8%) and its practicing workforce (66.7%), so PHM is skewing further female than pediatrics as a whole.17

Who gravitates here (the following reflects online and community perception and the broad pattern; plenty of people don't fit it): generalists at heart who love the breadth of pediatrics and don't want to narrow into one organ system or one clinic population; people who prefer bounded shift work to clinic continuity, who'd rather work hard for a defined block and then be fully off than carry a panel and an inbox indefinitely; team-oriented, systems-minded physicians drawn to quality improvement, patient safety, and hospital operations (PHM is a notably QI- and systems-heavy culture); and teachers, since academic PHM is built around residents and students. The common thread online is someone who likes acute variety and a fast diagnose-treat-discharge cycle, plus clean shift boundaries, over long-term clinic relationships.1218

The stereotypes. community caricatures, not facts. Each with a humanizing counterpoint:

  • "The generalist's subspecialty" / "didn't want to sub-specialize." The read online is that PHM is for people who love general peds but want inpatient acuity and shift boundaries. Counterpoint: practitioners push back that it's an affirmative choice for breadth and systems work rather than a fallback, and increasingly a defined, credentialed field with its own scholarship.18
  • "Shift work, so it's the lifestyle option." A perception with a real kernel, since the defined off-time genuinely is a draw. Counterpoint: the nights, weekends, and holidays are real, and the "off" blocks are earned with intense, circadian-disrupting "on" blocks. It's bounded, not easy.18
  • "Just a glorified senior resident." Because the day-to-day (rounding, admissions, floor management) resembles residency, outsiders sometimes undersell it. Counterpoint: attending hospitalists carry independent decision-making, medico-legal responsibility, QI/leadership portfolios, and teaching roles residents don't.18

A recurring practical note: the job market is strong and nationwide, since every children's hospital and most community peds units need hospitalists, so people frame PHM as flexible and findable, including near family.
