What they actually do
Strabismus is the surgical core, and it is not only a pediatric problem. Esotropia, exotropia, and the vertical and paralytic deviations are corrected by adjusting the extraocular muscles, and the operation is as much about measurement and prediction as about technique, since the surgeon is calculating millimeters of recession or resection against a measured angle. Adult strabismus is a substantial part of most practices, arriving from thyroid eye disease, cranial nerve palsies, trauma, and childhood strabismus that was never fully corrected. Because no other subspecialty claims adult strabismus, pediatric ophthalmologists carry it.
Amblyopia management is the field's defining medical work, and it is a race against development. Detection, refractive correction, patching or atropine penalization, and the follow-up that ensures a family actually complies, all inside a window that closes.
Retinopathy of prematurity is a screening and treatment service for premature infants in the neonatal intensive care unit, with a defined examination schedule, treatment by laser or intravitreal anti-VEGF injection, and consequences for missing the treatment window that make it one of the most medicolegally exposed activities in ophthalmology.
Pediatric anterior segment and glaucoma brings congenital cataract, where surgery must happen within weeks to prevent deprivation amblyopia and the visual rehabilitation continues for years, and congenital and juvenile glaucoma.

A day in the life: clinic-heavy and slow per patient. A pediatric eye examination takes considerably longer than an adult one, because the patient cannot report symptoms, will not cooperate on demand, and requires cycloplegic drops that need twenty to thirty minutes to work before the refraction can even begin. That single fact drives the field's economics. Operative days are outpatient lists of strabismus cases and examinations under anesthesia. ROP rounds happen in the neonatal intensive care unit on a fixed screening schedule, including weekends.
On call: modest but real. ROP screening does not pause, and a missed examination window has consequences. Pediatric trauma and acute presentations arrive through the emergency department.

Medical school (4 yrs) → PGY-1 intern year → ophthalmology residency (3 yrs, entered through the separate SF Match) → 1-year pediatric ophthalmology and strabismus fellowship → practice.12

How competitive is it?
This is one of the least competitive fellowships in ophthalmology, and the shortfall is well documented.
- 34% of pediatric ophthalmology fellowship positions went unfilled in 2022, higher than any other ophthalmology subspecialty except neuro-ophthalmology.4 ⟳
- Nearly half of positions have remained unfilled each year since 2018, and the vacancy rate ran between 32% and 52% across 2000 to 2015, so this is a long-standing structural problem rather than a recent dip.4 ⟳
- The overall fill rate has run around 75% over recent years, and roughly 40% of matched positions are filled by international graduates, many of whom do not remain in the United States, which means the effective domestic workforce contribution is smaller than the match numbers suggest.5 ⟳
- What predicts a resident choosing this field is relational rather than educational. A 2025 study of 66 US ophthalmology residency programs found that larger residency size, more pediatric ophthalmologists on the core faculty, and the presence of an in-house fellowship all substantially raised the odds a resident would pursue it, with an in-house program associated with 3.48 times greater odds. Total days of exposure and the timing of that exposure showed no significant association. Mentorship, not rotation length, is what recruits people into this field.4 ⟳
The honest read. Anyone who wants this fellowship can have it. The field's problem is that not enough people want it, and the reason named consistently in its own literature is reimbursement rather than the work.

Compensation — the robust version
This is the field's recruitment problem, and it is documented by the field itself.
The figure, with its date attached. Pediatric ophthalmologists averaged a gross salary of about $400,000 in the 2019 AcadeMetrics Benchmark Survey, which the Academy reported in 2023 and which is the only pediatric-ophthalmology line published anywhere.5 The parent field runs roughly $410,000–$490,000 on current surveys, with partners and owners near $610,000 and retina near $700,000.3 ⟳ Subtracting one from the other gives a number that looks more precise than it is, because a 2019 average against a 2025 survey is not a like-for-like comparison, so treat the size of the gap as unquotable and the direction as well established. The discount itself does not rest on the arithmetic: the field's own workforce literature names inadequate reimbursement as the deterrent, and the three mechanisms below are all documented independently of any salary figure.
Three mechanisms produce it, and none is about the value of the work.
- Volume per session is structurally low. A pediatric eye examination requires cycloplegic drops that take twenty to thirty minutes to act, a patient who cannot self-report, and a parent to counsel. Seeing fewer patients per day against a reimbursement system that pays per encounter is a mathematical problem no amount of efficiency solves.
- Strabismus surgery is poorly reimbursed and has been devalued. CMS has downgraded strabismus surgery codes, and the operation takes longer and pays less than the cataract extraction a comprehensive ophthalmologist performs in the same slot.5
- There is no cash-pay component and a Medicaid-heavy payer mix. Unlike oculoplastics with cosmetic work or cornea with refractive surgery, nothing in this practice is elective in the consumer sense, and children are disproportionately publicly insured.
The field's own literature is explicit about the consequence. Workforce analyses identify inadequate reimbursement as the major unresolved deterrent to recruitment, having addressed most of the other named factors.6 The result is millions of Americans without local access to a pediatric ophthalmologist. ⟳
What partly offsets it. The adult strabismus practice is better reimbursed than the pediatric one and is a real part of most jobs. Some pediatric ophthalmologists maintain a general ophthalmology practice, including adult cataract surgery, alongside, which substantially changes the arithmetic and is a recognized strategy.
Limited-data caveat: the $400,000 comes from the 2019 AcadeMetrics Benchmark Survey, quoted in an American Academy of Ophthalmology workforce article of April 2023, so the article is four years newer than the data and the data is now seven years old. MGMA and Doximity publish no pediatric ophthalmology line. Verify against actual offers, and ask specifically about the adult and general-ophthalmology component. ⟳

No pediatric-ophthalmology-specific wellbeing data exists. The satisfaction case is about permanence, and it is genuinely strong. Correcting amblyopia in a four-year-old gives that person functioning binocular vision for eighty years. Removing a congenital cataract in the first weeks of life is the difference between sight and lifelong deprivation. Few interventions in medicine have a return measured on that scale, and practitioners name it consistently as the reason they chose a field that pays what this one pays.
The distinctive stress is the deadline. Amblyopia treatment has a window, and a child who presents late, or whose family cannot sustain patching, arrives at permanent loss that was preventable. Carrying that, repeatedly, across a panel where compliance depends on households under strain, is the specific weight of this field.
Retinopathy of prematurity is its own category of anxiety. The screening schedule is defined, the treatment window is narrow, the consequence of missing it is blindness in an infant, and the medicolegal exposure is among the highest in ophthalmology. Many pediatric ophthalmologists describe ROP as the part of the job they would most like to hand to someone else.

Strabismus also attracts a particular analytic temperament, because surgical planning is a quantitative prediction problem. As always, plenty of people in the field do not fit any single mold.

- "More training to earn less." The field's own shorthand, and accurate: an extra year for a salary below comprehensive ophthalmology.5
- "You have to like children." True, and incomplete, since adult strabismus is a large share of most practices.
- "The eye exam takes an hour." Close to literal once cycloplegia is included, and it is the root cause of the compensation problem rather than a complaint about pace.
- "ROP is a lawsuit waiting to happen." Overstated and not baseless. The screening schedule is unforgiving and the consequence of a missed window is severe.

The FLI angle — Pediatric ophthalmology for first-gen, low-income & immigrant students
Where it fits FLI realities well:
- Geographic freedom is real and unusually valuable here. The field is short of people nearly everywhere and millions of Americans lack local access, so a graduate genuinely chooses where to live.6 For a student who needs to stay near family, that is worth a great deal.
- The fellowship is not a barrier. With a third of positions unfilled and a fill rate around 75%, anyone who wants this training can get it.45
- The international-graduate route is genuinely open at the fellowship stage, with roughly 40% of matched positions going to international graduates, which is far higher than ophthalmology's residency IMG figure of about 2%.53
- The income is still strong in absolute terms at roughly $400,000 on the 2019 survey figure, even though it is below the parent field.5
- The patients may be your own. Preventable amblyopia falls hardest on low-income and non-English-speaking families, where screening and patching compliance break down for practical reasons. A physician who understands why a family cannot make the follow-up appointment is doing something the field badly needs.
Risks to name honestly:
- The residency remains among the least FLI-accessible gates in medicine, at roughly 34% DO senior match rate and about 2% IMG of matched positions, through a separate early SF Match.3 That is the barrier, and the fellowship's openness does nothing about it.
- This is the only ophthalmology fellowship that reliably lowers your income, and the mechanism is structural rather than negotiable.5 Model it against comprehensive ophthalmology before committing, and read it alongside the Money section on debt and specialty choice.
- PSLF fits academic practice and not private groups, the same fork as the rest of ophthalmology.
- Plan the combined practice deliberately. Keeping adult cataract and general ophthalmology alongside is the recognized way to make the economics work, and it is a contract-time decision rather than an afterthought.
Bottom line for FLI: an open fellowship, genuine geographic choice, and work with some of the longest-lasting benefit in medicine, attached to the one ophthalmology subspecialty that costs you money. If you can reach the residency, this is a field that will take you, need you, and let you live where you want.
