What they actually do

Pediatric rheumatologists diagnose and manage autoimmune, autoinflammatory, and inflammatory diseases of the joints, connective tissue, and immune system in children and adolescents.1 The core diseases are juvenile idiopathic arthritis (JIA) and other chronic childhood arthritis, childhood-onset lupus and the connective-tissue diseases (scleroderma, dermatomyositis, Sjögren, mixed connective tissue disease), the vasculitides (Kawasaki disease, IgA vasculitis, ANCA-associated, Behçet), the autoinflammatory and periodic-fever syndromes (PFAPA and the monogenic periodic fevers), and chronic non-infectious uveitis, plus MIS-C evaluations and chronic or amplified musculoskeletal pain.1 Because almost all of these are chronic, the field is built on longitudinal continuity (you follow the same medically complex children for years, sometimes into young adulthood) and multidisciplinary care (close loops with ophthalmology for uveitis, plus dermatology, orthopedics, nephrology, and PT/OT).1

The core skill is diagnostic synthesis: being the detective on the febrile, multi-system, undifferentiated child other specialists couldn't figure out, and then managing years-long immunosuppressive and biologic therapy. This is a thinker's specialty, not a proceduralist's: the hands-on work is limited mostly to joint injections/arthrocentesis and a growing use of point-of-care musculoskeletal ultrasound.1

Representative procedures: joint aspiration (arthrocentesis) · intra-articular corticosteroid injections (often with sedation/ultrasound guidance) · musculoskeletal ultrasound · ordering/interpreting MRI, X-ray, and lab studies · coordinating (with other services) skin/muscle biopsy and other diagnostic workups. Much of the "procedural" load is really infusion oversight: supervising and adjusting biologic and immunosuppressive infusions (infliximab, rituximab, cyclophosphamide, IVIG).1

A week in the life: Predominantly a Monday–Friday, clinic-based life. The bulk is longitudinal continuity clinic, with new consults and, more often, return visits for chronically ill children whose disease is monitored and titrated over months to years.2 Layered on top: inpatient consults to the wards, PICU, and ED (new-onset lupus, systemic JIA with macrophage activation syndrome, vasculitis, fevers of unknown origin, MIS-C), infusion-center oversight, in-clinic joint injections, and, because ABP certification requires scholarly work, protected time for research, QI, or teaching for many practitioners.23 Self-reported hours cluster around 40–55/week depending on research and clinic load; the days are cognitively dense, but the shape of the week is predictable.2


The training path & time to completion

Medical school (4 yrs) → general Pediatrics residency (3 yrs) → Pediatric Rheumatology fellowship (3 yrs) → board-eligible with the American Board of Pediatrics (ABP) in Pediatric Rheumatology.3 This is a two-step entry: you match into pediatrics residency straight from medical school, then apply again during residency to a 3-year fellowship through the NRMP Medicine and Pediatric Specialties (Fall) Match.34 (The field is also reachable via a Med-Peds residency.)

  • Fellowship (3 yrs): ACGME-accredited (RCPSC in Canada), with a required scholarly-activity and research component. The three years typically combine core clinical training (concentrated early, plus continuity clinic throughout) with substantial protected time for research/scholarship in the later years.3
  • Board: the American Board of Pediatrics (ABP), whose subspecialty certification in Pediatric Rheumatology requires ABP general-pediatrics certification first, completion of the accredited 3-year fellowship, and passing the sub-board exam.3

How competitive is it?

The single most important thing a premed should understand about this field is that getting in is not the challenge. Pediatric rheumatology is chronically, severely undersubscribed, one of the least-filled fellowships in all of American graduate medical education and the near-opposite of adult rheumatology, which fills ~99%.4 The accessible front door is general pediatrics residency, one of the more open, DO/IMG-friendly routes in medicine, and the fellowship step, uniquely, is easier still because there aren't enough applicants to go around.64

  • The shape is the finding. The fill rate has fallen in four of the last five cycles, from 69.2% to 54.5%, and the number of fellows entering the field has not risen once: 27, 27, 32, 27, 24. That is a slow, consistent decline rather than one bad year, which is what makes the access crisis below a structural problem rather than a blip.4
  • About half of all pediatric rheumatology fellowship positions go unfilled every year, and roughly half of programs fail to fill.4
  • The higher percentage in 2026 (54.5% vs. 49.1%) is misleading: programs cut offered positions (55 → 44). The ACR explicitly noted that if 2025's position count had held, the 2026 fill rate would have declined, so both capacity and applicant interest are shrinking.4
  • Applicant-to-position ratio is well below 1.0. There are fewer applicants than seats. A pediatric-subspecialty-wide analysis put rheumatology at ~0.62 applicants per position (2024), among the lowest of any pediatric subspecialty; only ~24–33 applicants rank the field each year, and interest is declining (top-choice applicants fell ~14% since 2022).74

The honest read: the difficulty here is not the match but the pipeline: too few pediatricians choose the field, largely for the compensation and training-length reasons below. For a student building steadily toward it, the path is unusually forgiving. Program leaders describe the steady trickle of applicants as "insufficient…for our current needs, much less for the future."4


Compensation — the robust version

Peds rheum pay is moderately documented but genuinely uncertain, because it's a tiny, overwhelmingly academic field with small survey samples and inconsistent definitions (base vs. total comp). Lean on the pattern, not any single point estimate.5

Where it sits, in one blunt line. Pediatric rheumatology is one of the lowest-paid physician fields in the United States, full stop, and it pays less than general pediatrics despite three additional years of low-paid fellowship.5

National number. Doximity 2025 (2024 data): ~$231,574, about $33,656 below general pediatrics ($265,230).5 A job-board aggregator (ZipRecruiter) estimates ~$313k, but academic and children's-hospital reality generally runs well below that; treat job-board figures as rough and high.2 A defensible "typical" figure is near or just above $231k, and below general peds. ⟳

Doximity's commentary groups the below-general-peds outpatient fields under the mnemonic "READING": Rheumatology, Endocrinology, Adolescent medicine, Developmental-behavioral, Infectious disease, Nephrology, and Genetics. All are cognitive, non-procedural, and therefore low in the wRVU productivity that drives physician pay.5

Why so low (structural). These fields generate few high-RVU procedures; the work is almost entirely cognitive and outpatient, so billing productivity is intrinsically low. Pediatrics also carries a Medicaid drag, since more than half of US children are on Medicaid, which reimburses below cost.5 And critically, peds rheum has no infusion/"buy-and-bill" income lever: the biologics are delivered in hospital infusion centers, not physician-owned clinics, so the revenue that pads adult rheumatology income is essentially unavailable.5

Academic base, because it's an academic field. More than 72% of pediatric rheumatologists work at teaching hospitals, so compensation is an academic-salary story rather than a private-practice one.8 A peds-rheum-specific assistant-professor base wasn't published as a clean number (limited data), but the comparable "READING"-tier peds ID assistant professor was cited at $147k–$158k, or "less than a new computer-science bachelor's grad," so expect roughly a $150k–$180k academic base band, rising modestly with rank, tenure, and division-chief/leadership roles, but with ceilings well below procedural peds.5

Geography / urban vs. rural. State- and region-level peds-rheum tables essentially don't exist (the workforce is too small to break out; limited data). Structurally, the field is concentrated in urban academic children's hospitals; there is effectively no rural private-practice market. This is an access-desert specialty where rural children travel to the specialist, rather than one with a rural pay premium.58

The adult-rheumatology contrast. Adult rheumatology is itself lower-middle on the IM pay ladder, yet it clearly out-earns the pediatric field: ~$284k (Medscape 2026) to ~$325k (Doximity 2025), a defensible "typical" ~$300k, plus the in-office infusion revenue peds rheum can't access. Net: an adult rheumatologist out-earns a pediatric one by roughly $55k–$90k+, for shorter training (a 2-year IM fellowship vs. a 3-year peds fellowship).5

The number that colors everything is lifetime earnings. Peer-reviewed work shows that most pediatric subspecialties, including rheumatology, carry a negative lifetime financial return versus simply practicing general pediatrics: you spend ~3 extra fellowship years at trainee pay for a salary that often doesn't exceed general peds. In one analysis, 12 of 15 pediatric subspecialties had negative lifetime returns, and the best-vs-worst gap across pediatric paths widened to over $2.3 million (2018–2019 data). Lower pay also correlated with worse workforce growth and access, and peds rheum is the textbook case.9

Compensation bottom line: ~$231k, below general pediatrics and near the bottom of all physician pay, after three extra fellowship years that, on average, reduce lifetime earnings versus stopping at general peds. Almost entirely academic; no infusion lever. This field is chosen for the patients, the puzzle, and the mission rather than the money, which is worth internalizing before you fall in love with it.


Lifestyle & the schedule bargain

The most-cited pro of peds rheum: it's one of the most schedule-controllable fields in all of pediatrics.2 The practice is predominantly outpatient, Monday–Friday continuity clinic, and the field is widely regarded within pediatrics as a "sane call" subspecialty.

  • Call is light and low-intensity. Most patients are outpatient and chronic; acute overnight emergencies are relatively uncommon versus NICU, PICU, cardiology, or heme/onc. Inpatient consults exist (new lupus, systemic JIA/MAS, vasculitis, fevers), but the intensity of call is usually low. The honest caveat: because groups are often small (see the workforce crisis), call frequency per person can run higher at a lean center, even if each call is rarely a crisis.2
  • Schedule control is high, and the field is among the most compatible in pediatrics with part-time and flexible scheduling. The workforce literature explicitly names work-life balance and part-time availability as recruitment/retention levers.28
  • Continuity is deep. You become a fixture in families' lives across years of a child's development, which is the payoff for people who find meaning in relationships, and a real emotional weight to carry over the long haul.2

Lifestyle rating: 4.5/5. Among the most controllable in pediatrics. The honest caveat is that the daily drag here is prior-authorization battles to get expensive biologics approved, understaffing, and waitlists you can't clear, rather than chaos or hours.


Wellbeing — the part to take seriously

Meaning runs unusually high. The dominant theme in the field's identity is that biologics turned peds rheum into a "results" specialty. Kids who a generation ago faced joint destruction, wheelchairs, uveitis-driven blindness, or organ damage now frequently reach remission on TNF/IL-1/IL-6 blockade. Practitioners describe watching a limping, exhausted child return to school and sports; combined with an enormous unmet need (you are often the only pediatric rheumatologist a family could reach), the sense of purpose is a repeatedly cited reason people stay.2

The burnout drivers here are unusual, and the schedule is not among them. They are being under-staffed (long waitlists, being stretched thin by demand), insurance and prior-auth friction (a real, recurring tax precisely because the effective drugs are costly), and the isolation of practicing solo or in a small group far from colleagues.2 Dissatisfaction, when it appears, clusters around compensation, isolation, and demand rather than around the medicine.2 Nobody has published a would-choose-again figure by specialty since about 2019, so there is none to quote.

What is measured here, and what is a reading of the schedule. This field has long been described as lower-risk than the high-acuity, high-call pediatric specialties, and that is a reading of how the work is built rather than a measured comparison: the clinic runs to schedule, the continuity is long, overnight call is light, and emergencies are rare. No survey has tested it. Neither instrument publishes a pediatric rheumatology row, and neither publishes an adult rheumatology row either. Medscape's 2024 report names only its ten highest and ten lowest specialties and rheumatology appears in neither, which is where the field's "mid-pack" reputation comes from rather than from a printed figure.10 So the only candidate proxy carrying a published number is general pediatrics, at 51% against that survey's own 49% all-physician average, and that is what this page reads, because the patients, the referral pattern, the children's-hospital setting and the payer mix here are pediatric while the adult version of the same immunology sees a different population in a different building. Read the lower-risk statement as a comparison of hours and acuity inside pediatrics rather than as a claim that this field measures below the physician average, which nothing published can show.10

Career longevity is a genuine strength. The controllable schedule and cognitive (rather than physically punishing) nature support long careers. Retirements of an aging workforce are in fact part of the shortage problem, i.e., people tend to stay in it.2

The FLI angle — Pediatric Rheumatology for first-gen, low-income & immigrant students

This field has a genuinely unusual FLI profile: a low financial ceiling, but high accessibility, high job security, high mission-fit, and unusually strong loan-forgiveness compatibility. The honest work here is framing money vs. meaning with clear eyes.

Where peds rheum fits FLI realities well:

  • The door is wide open, and this is one of the more accessible fellowships in medicine. You enter through general pediatrics residency (an open route, and an unusually IMG-friendly one, though its DO share only matches the all-specialty average), and then apply to a fellowship that doesn't fill about half its spots every year. In 2025, ~30% of matched fellows were DOs and ~22% were IMG/foreign, a real and non-token lane driven purely by undersubscription. This is the opposite of fighting through a pedigree-gated, hyper-competitive match.64
  • The loan math can be rescued, and this is the key FLI hack. Peds rheum jobs are overwhelmingly at academic medical centers and nonprofit (501(c)(3)) children's hospitals, the ideal employers for Public Service Loan Forgiveness (PSLF): 10 years of income-driven payments at a qualifying nonprofit wipes the federal balance tax-free. For someone with large loans and a modest salary, PSLF can neutralize much of the "negative return" problem on the federal balance, making this structurally one of the best PSLF fits in medicine. The geographic gaps also open NHSC / state loan-repayment options in underserved areas.2 The word federal is doing real work in that sentence, and on this page more than most. Since July 2026 federal borrowing for medical school stops at $200,000, and school costs more than that almost everywhere, so a student starting now graduates with a private loan sitting beside the federal one and no program forgives it.
